Histološka analiza obproteznega tkiva pri okužbi ortopedskega vsadka

Pri diagnostiki okužb sklepnih protez (OSP) ima histološka analiza pomembno vlogo. Prisotnost nevtrofilnih granulocitov v obproteznem tkivu je povezna z diagnozo OSP. Histološki kriterij, ki se uporablja v številnih centrih, je ugotavljanje več kot pet nevtrofilnih granulocitov v petih mikroskopskih vidnih poljih 400-kratne povečave. Uporabnost histološke diagnostike okužb sklepnih protez je z vidika specifičnosti, senzitivnosti ter pozitivne in negativne napovedne vrednosti odvisna od histološke tehnike in analiziranega vzorca, od pražnih vrednosti, ki si jih postavimo, in seveda od pacienta. Zavedati se moramo tudi pomanjkljivosti histološke analize; zlasti okužbe z nizko virulentnimi bakterijami lahko dajejo lažno negativen rezultat, medtem ko je rezultat pri pacientih s periprotetičnim zlomom ali revmatoidnim artritisom lahko lažno pozitiven. Ključna je dobra komunikacija med patologom in ortopedom, ki bistveno zmanjša pomanjkljivosti histološke diagnostike OSP.

How to estimate kidney function in 2023

In practice, GFR is estimated (eGFR) from endogenous marker concentrations. However, current eGFR approaches still lack accuracy. Conversely, exogenous marker clearance, i.e., measured GFR (mGFR) is the most accurate method for evaluating kidney function; however, standardized protocols are lacking. Inulin clearance has been considered the reference method for determining mGFR. Given the limitations of serum creatinine and inulin, non-radioactive and radioactive exogenous mGFR markers are the most accurate options for evaluating GFR. However, mGFR procedures are complex and have a degree of error. Limited Cr- ethylenediamine tetraacetic acid availability, lacking certified reference iohexol (off-label IVD) materials, and lacking uniform protocols all question the merits of mGFR methods as a golden standard for evaluating kidney function. Furthermore, regulatory issues are narrowing the clinical use of mGFR. In children, the modified Schwartz equation can only be used in combination with enzymatic creatinine assays, and thus there is a growing tendency to apply cystatin-C-based approaches in pediatric nephrology.Creatinine-based eGFR equations have their limitations, particularly regarding imprecision. Creatinine-based equations have lower accuracy in ethnic groups other than white and Afro-American groups. Cystatin C is an alternative: eGFR has a higher accuracy than that of eGFR and eGFR. Cystatin C equations perform as well as the CKD-EPI 2012 equation. The combined equation is not independent of eGFR and requires specifying ethnicity. The CKD-EPI 2012 eGFR equation can be used without specifying ethnicity but is not more accurate than the CKD-EPI 2009 eGFR equation. Despite newer American recommendations, the European Federation of Laboratory Medicine still adheres to this equation.

Diagnostična uporabnost določanja koncentracije lipoproteina(a) in
genetske variabilnosti gena LPA

Koncentracija lipoproteina(a) (Lp(a)) je določena genetsko in je neodvisni dejavnik tveganja za srčno-žilne bolezni. Lp(a) je s holesterolom bogat lipoproteinski delec, ki nastaja v jetrih. Sestavljen je iz apolipoproteina B100 in apolipoproteina(a) (apo(a)), ki sta povezana z disulfidno vezjo. Apo(a) kodira , ki je eden najbolj polimorfnih genov pri človeku, saj je bilo identificiranih že več kot 40 genetskih variant. Koncentracija Lp(a) je v 90 % genetsko določena preko variant v genu in je v aterogenem območju (> 300 mg/L) prisotna pri 20 do 30 % populacije. V zadnjem desetletju je prišlo do velikega napredka pri razumevanju mehanizmov, ki so vpleteni v sintezo in presnovo Lp(a), kar je izboljšalo tudi razumevanje patofiziologije Lp(a) in dejavnikov, ki vplivajo na njegovo koncentracijo. Kljub temu da določanje Lp(a) še ni standardizirano, s kliničnega vidika zadostuje za prepoznavanje posameznikov s povečanim tveganjem za srčno-žilne bolezni. Pri teh posameznikih je ključno zmanjšanje tveganja, zato so v postopku razvoja nove, obetavne terapevtske možnosti za zniževanje koncentracije Lp(a), ki so usmerjene proti mRNA gena . Namen prispevka je predstavitev najnovejših ugotovitev o strukturi, sintezi, presnovi, patofizioloških mehanizmih in terapevtskih možnosti za zniževanje koncentracije Lp(a) ter izpostaviti izzive pri njegovem določanju v kliničnih laboratorijih.

Women social and health inequalities in the post-pandemic era

The COVID-19 pandemic has strongly affected all walks of life and has devastated lives globally. The pandemic has further increased economic and social inequality in a vulnerable society. As per The World Economic Forum’s Global Gender Report 2021, it is estimated that the global economy has been set back by up to 39 years during the pandemic era. Furthermore, healthcare access has been disrupted for women and girls, a subject which has been ignored for a long time. Precipitating factors further led to a rise in gender-based violence and sex-based marginalization. Health inequalities related to economic status and social and cultural environments are increasing. These differences are attributable to social disparities between men and women, which often assume different functions and tasks and are therefore exposed to different morbidity factors. Others are innate to biological, genetic, hormonal, and metabolic dissimilarities or acquired (environmental) dissimilarities. It appears that both social and biological differences play a role; however, the border between the two is difficult to delimit. Thus, long-term measures are needed to balance between professional and personal life for the economic independence of women.
